Since this was a development machine I specified a single BizTalk service account for all purposes in the BizTalk Server Configuration application.
The account was a member of these groups:
- BizTalk Application Users
- BizTalk Isolated Host Users
- IIS_IUSRS
- SSO Administrators
- Users
